2007 NCCATS Tournament Rules & Regulations Acceptance and Entry Form

I (We) by signing hereby release NCCATS and all participating sponsors of this tournament from any loss of property, bodily injury, or loss of life that may occur in this tournament.  I understand that selected contestants must take and PASS A POLYGRAPH TEST or VOICE STRESS TEST to qualify for the receipt of any prize and order to verify compliance with all tournament rules.  Only one test per claim will be given.  I release NCCATS and all participating sponsors from any liability resulting from the decision of the examiner, agree that the decision of the examiner is final, and agree to abide by the decision of the examiner.  I agree that any mention of my name or photographic likeness may be used ny NCCATS for promotional purposes.
